FORM 4 - UCCI Scholarship Application
UCCI has been fortunate in the fact that many current and former campers have contributed to establish a camper scholarship fund. The generosity of these campers has helped make camping experiences at Pilgrim Center and Moon Beach Camp possible for dozens of campers for more than a decade.
The UCCI Board, as the stewards of these funds, awards scholarships on the basis of need. Download this form for more information or to apply for financial assistance.

FORM 5 - Health Forms
A completed health form is required for all campers.
For campers under 18 years old the form includes two sections:
- Health History (the front and the top of the back of the form) which must be completed by a parent or guardian before the child can attend camp.
- Release Form (the bottom of the back of the form) which must be signed by the camper and by the camper's parent or guardian.
