FORM 3 - UCCI Scholarship Application
UCCI has been fortunate in the fact that many current and former campers have contributed to establish a camper scholarship fund. The generosity of these campers has helped make camping experiences at Pilgrim Center and Moon Beach Camp possible for dozens of campers for more than a decade.

The UCCI Board, as the stewards of these funds, awards scholarships on the basis of need. Download this form for more information or to apply for financial assistance.

FORM 4 - Child Immunization Record
Required for all campers under the age of 18.

FORM 5 - Health Form
A completed health form is required for all campers.

The form includes two sections:

-Health History

-Release Forms - which includes the Participant's Statement, Medical Authorization, and Photo Release. All three require a signature by the camper (and Parent/Guardian if under 18 years old).
