Registration Form
Use this form to register for all UCCI events. You can print and mail of fax it to
either site -- or forgo the form and call the site to register by phone.
Health Form
A completed health form is required for all campers under 18 years old. The form includes
two sections:
- a health history (the front and the top of the back of the form) which must be completed
by a parent before the child can attend camp.
- an release form (the bottom of the back of the
form) which must be signed by the camper and initialed and signed by the camper's parent or
guardian.

Medication Form
Parents should complete this form to better facilitate our health services staff
in dispensing medications that campers bring to camp with them.

This information is provided for your information and reference. We
always encourage parents to carefully assess your child's health before you send
them to live in close community at camp -- especially if they have a contagious
disease like the mumps!
Liability
Release Form
This form is specific to the Jr Hi and Senior High Adventure camps. Parents are
required to initial and sign this release before campers can participate in the
Adventure camps. This form is occasionally used for other camps where
transportation is provided for off-site activities such as the Sr Hi Sail Camp
and the Harry Potter Camp.)
Packing
List
Suggested packing list for youth camps. You may need to
pack more or less. Please, please mark all
clothing and other personal articles you send to camp with your child. Untold hundreds of
dollars in clothing, towels, Bibles and more are left at camp each summer. Mark your
belongings and help us make sure you take it all home with you.
Construction Detour Information
We are fortunate in the fact that the state invests in the roads in the
state -- they, in so many ways, are the lifeblood of our common life.
However, this is little consolation if you are delayed by construction or you
are lost in a construction detour!
We can't be informed of every construction detour on the way to camp, but we
know the people who do know all about these things! You can visit
the Wisconsin Department of Transportation web site to find out if there is
construction along the route you will be traveling.

Pilgrim Center is easy to find -- centrally located to most major population centers in
Wisconsin. Pilgrim Center is:

Driving Directions to Moon Beach
Camp
Located in the beautiful Northwoods of Wisconsin,
Moon Beach is just 30 miles north of Rhinelander between Eagle River and St Germain.
Please note: The
directions on mapquest and most other mapping web sites all provide great
directions until you turn off Highway 70. You can use mapquest to
get within three miles of camp -- and then please use the directions
provided here

UCCI Scholarship Application
UCCI has been fortunate in the fact that many current and former campers have
contributed to establish a camper scholarship fund. The generosity of these
campers has helped make camping experiences at Pilgrim Center and Moon Beach
Camp possible for dozens of campers for more than a decade.
The UCCI Board, as the stewards of these funds, awards
scholarships on the basis of need. Download this form for more information

Background Disclosure Form
All staff -- paid and volunteer -- are required to complete and submit this
form to the UCCI office before they are permitted to work with our campers in
our programs.

This document compiles responses to a "what was it like when you were at camp"
survey of our managers and directors at our camps. This book provides
wonderful insight into the individuals who gave of their time and talents over
the years -- and a glimpse into how much camp has changed over these years!
